This role will be based out of our Assisting Hands Home Care Fremont office located near Fremont, California. The Community Liaison Representative is responsible for maintaining relationships with new and existing clients and community partners in the healthcare industry. They will also perform client intakes, assessments, and regularly scheduled visits/wellness check-in calls. This individual is expected to act as an extension of our company's customer service department to ensure that the client's needs are met as per our company's standards.
The Community Liaison Representative will possess excellent communication skills, organization, and superior customer service. They will perform community outreach activities to strengthen Assisting Hands' relationships with healthcare organizations, connected networks, and social service centers. At the same time, they'll help in the development of marketing materials and strategic plans for outreach. The Community Liaison Representative ensures that our clients and their families are satisfied and have a good overall experience with our services and act as the primary point of contact for all clients.
Location: In-person role at 39675 Cedar Blvd, Suite 180, Newark, CA 94560 (not a work from home position).

Responsibilities- Generating regular streams of new business by building relationships with facilities/communities and representing the organization at networking events.
- Conducting client intakes, assessments, and regularly scheduled visits/wellness check-in calls.
- Developing and maintaining the client care plan, creating extraordinary client experiences and communicating with clients, families, care partners and referral sources.
- Serving as a mentor to caregivers and supporting their care delivery.
- Providing after-hours support for our clients, referral partners and prospective clients - this support may include email, telephone, scheduling and/or home visits.
- Help out with hiring, scheduling tasks as needed.

- Prior client management, sales and or marketing experience, preferably in the home care industry

Qualifications- A bachelor's degree in business, marketing, communications, or a related field required;
- Previous experience in Home Care or Home Health is a plus but not required;
- Can do attitude, high energy and motivated
- Excellent communication, interpersonal, and problem-solving skills;
- Ability to build rapport with clients, families, and healthcare professionals;
- Ability to document conversations, organize reports, and conduct presentations;
- Ability to prioritize and manage multiple tasks and meet deadlines;
- Proficient in computer software programs - Google suite, CRM, MS Office (Word, Excel, PowerPoint);
- Up-to-date car insurance, vehicle registration, and valid Driver's License;
- Successful clearance of background checks with relevant work-related references.
Compensation: $45,000.00 - $65,000.00 per year
