A day in the life!
You will be responsible for the following:
- Conducting quality assurance documentation reviews of assigned records. This includes:
- OASIS for documentation inconsistencies and accuracy
- Plan of treatment review for accurate reflection of patient’s condition and skilled interventions
- Medication profile for relevance to interventions, goals and coding sequencing
- Appropriate physician’s orders
- Care planning as it relates to assessment and regulatory requirements
- Conducting reviews of coding and providing recommendations based on current coding guidelines
- Providing written feedback related to documentation and coding
- Conducting other clinical record audits as assigned
- Maintaining proficiency/knowledge of OASIS data set items, home care reimbursement and compliance as related to OASIS regulations and requirements.
- Maintaining Coding Certification
- Developing and maintaining knowledge of software systems utilized by various agencies
- Identifying and communicating concerns, trends, ideas and possible solutions to improve processes/procedures
- Proactively identifying and assisting with client concerns.
- Participating in client discussions

Required Qualifications:
- 2-4 years of related work experience
- Coding Credentials (HCS-D or equivalent)
- OASIS Credential (COS-C or equivalent)

Preferred Qualifications:
- Previous Home Health and Hospice experience
